Transferring to Downstate Medical Center?

Transfer Your Sponsored Research to Downstate

When an Investigator transfers to another institution, Downstate will generally relinquish a grant for transfer to the new institution provided that the transfer is consistent with the policies of the granting agency. DSP should be contacted as soon as possible after acceptance of a new position - transfer of federally funded awards can take 6-8 weeks or more.

The following must be submitted:

From Downstate

PHS 3734 Relinquishing Statement (for PHS funded awards) or comparable form from other federal or non-federal sponsors

Final Invention Statement (completed by PI and countersigned by DSP)

Financial Status Report

From new institution

A completed research grant application PHS398 with "CHANGE OF GRANTEE INSTITUTION" typed across the face page. The application should be a revised version of the current grant which updates information relating to the new institution.

The narrative section is an updated progress report noting any changes that may occur in the program as a result of the move

A new face page, resource and environment statement, budget pages based on the amount to be transferred, budget justification, F&A cost calculations and other support pages, all reflecting information of the new institution.
